A study on the treatment of primary dysmenorrhea by using a blade needle to release myofascial trigger points under ultrasound guidance

Abstract: Background Primary dysmenorrhea (PDM) is a common gynecological disease in clinical practice, which tends to occur in young women. To date, there is no unified and effective method for treating PDM, which has thus become a worldwide medical challenge. In recent years, some researchers have adopted the approach of inactivating myofascial trigger points (MTrPs) in PDM patients, achieving considerable therapeutic effects, though relevant studies remain scarce. Our research team found in a preliminary small-sample study that releasing abdominal MTrPs can indeed alleviate pain in PDM patients and reduce recurrence, and that the release under ultrasound guidance was more precise with better therapeutic effects. Methods/Design The study will be conducted on patients with PDM who are admitted to our department between January 1, 2024 and December 31, 2026. The patients will be randomly assigned to two groups. The myofascial trigger points group will receive blade needle therapy at the MTrPs in the abdomen. One treatment will be administered on the day when the pain is most severe during menstruation (usually the first day), and it will be continued for 3 consecutive menstrual cycles. The ordinary acupuncture group will receive routine acupuncture treatment which is initiated 3 days before the onset of menstruation and discontinued when dysmenorrhea ceases, with once-daily administration for 3 consecutive menstrual cycles. The primary outcomes are Visual analogue scale(VAS), Cox menstrual symptom scale(CMSS), Prostaglandin F2α(PGF2α), Prostaglandin E2(PGE2)and Hemodynamic indices of the uterine artery(including RI, PI, S/D). All clinical assessments will be evaluated before and after treatments. Discussion This experiment will help to objectively evaluate the clinical effectiveness of ultrasound-guided blade needle release of myofascial trigger points in the treatment of PDM.
